On your first point - We have a "Static" SuperDOM which is exactly what you are looking for but this comes at a cost of $0.10 per side since its a patented interface. Instead of the bid/ask moving rapidly in fast moving markets, the price is fixed and the market climbs up and down the ladder. If you wish to use this to trade live, send an email to sales at ninjatrader dot com with a request.
On your second point - Have you checked out Single Click Order Modification? You can set this property to true in the SuperDOM properties menu. This might help.
