Regularly cancelled or adjusted orders and random multi-orders

I've been getting regularly cancelled orders when running live that I never get when running in playback. I also get multi orders that cause issues like ordering more than I have in the account, or reversing a position when trying to exit. I'd like it if someone could take a look at my code or explain what's happening from the logs. I happen to be off all day tomorrow and will be able to do a screen share if anyone is available to take a look.
