I decided to change tactics and started using SubmitOrderUnmanaged method instead. This seems to be solving most of my issues.
For others looking to solve this issue as well: I had to set EntriesPerDirection and EntryHandling during State.Set Defaults.
EntriesPerDirection = 5; EntryHandling = EntryHandling.AllEntries;
I also saw that I had a ton of orders piled up from testing in accounts which weren't showing up in my Orders tab. To fix this, I went to the Accounts tab, left clicked on the account, selected Edit Account and reset the data to clear all old orders. I did this for each account I found any orders and that has finally cleared out the old data.
