What are the most common reasons for "overfills"?

In-flight executions and bracket orders are some of the more common scenarios where you can have an overfill.

could you explain the in-flight executions and elaborate a bit more overall perhaps?

also...it seems to happen on range, renko, unirenko, tick, etc type bars and not standard bars like 1 min, 5 min, etc.

Birdog,

An In-flight execution would occur if you placed a trade quickly to close a position while a prior order to close the same position was filled while in transit. This can cause both orders to be filled and putting you into an unwanted opposite position

With the bracket orders if the market is moving fast up and down, you can potentially end up having both orders fill rather than the other cancelled out.
