over fill after system placed order but trade stuck in the working state

My system was live last week it placed an order however the trade was stuck in a working state according to ninjatrader, when I looked in my tdameritrade account the trade was filled.
Days later when the system tried to exit nothing happened since that order was in a working state still and failed to exit.
Luckily I was home to exit manually, when I exited this caused an over fill and the system to be disabled.
How can I ensure that ninjatrader will fill my orders?

When this occurs once you have closed the position with your broker please repair your database to remove the order from NinjaTrader. To repair your database please go to Tools -> Options -> Data -> and click Repair DB. If this does not remove the order then the order will be removed when your broker resets their servers.

This is no expected behavior and the orders should update in NinjaTrader when filled. So I may investigate this matter further please send me your log and trace files for today so that I may look into what occurred. You can do this by going to the Control Center-> Help-> Mail to Support.
