Cancel order error

Hey,

Ok so I've dug around a little to find information on this error:

submitted a cancellation request for order ID "blah blah blah" has not been confirmed cancelled. Please check your account order and positions.

I found like 2 posts on it nothing really useful. My questions are
1) What's meaning/ logic for that error? You submit a cancel to the broker and the broker doesn't confirm it?
2) What could be the possible reasons to cause it, or could it be anything and the only way to tell is to study the log and trace files?

• You will find the log file in the Documents > NinjaTrader 7 > Log folder.
• The log file will be named "log.YYYYMMDD.txt"

• You will find the trace file in the Documents > NinjaTrader 7 > Trace folder.
• The trace file will be named "trace.YYYYMMDD.txt"
