Please check this, does not fire all the times when it should

I have created this buy signal using the condition builder. Then i backtested it on ESz9 using a volume setting of 12000. Then i added the volume indicator to the chart so i can see all the relavent events. Here is my understanding of when it should draw an up arrow:

Anytime the stochasctics K crosses above D within the past 5 bars AND
Anytime the Stoch K is less than 20 within the past 5 bars AND
Anytime the volume is > 12000 within the past 5 bars.

In other words any one of these conditions can occur within 5 bars of the others.

Looking at the chart, it fires off an arrow at some of the events but not others even though according to the condition of the indicators it should have.

Can you please explain what the problem is and how to fix it. Thanks.

The only way to know what is happening is to debug and print out values of each of the various conditions on each bar and compare this data to your chart.
