Form with Buttons Example

Hi,

would someone be able to direct me to an example of adding a custom form to an indicator/chart? I want to be able to add buttons to a form that will trigger events in the indicator, basically for backtesting.

I'm sure I have seen an example in this forum somewhere, but now for the life of me I cannot find it.
