Error - "an overfill was detected on order"

One of our automated strategies today had a long position and the stop loss was hit.

This stop was executed twice (see attached screen captures) and then Ninjatrader executed a flattening order and shut the strategy down with the error "An overfill was detected on order xxxxxxx"

Any ideas would be greatly appreciated.
