Commission per monthly volume?

What exactly is a commission level? Is it a monthly level or per trade level? For example, I have this monthly commission schedule with my futures broker:

Volume: Comm per side:
1-200 $2.11
201-1k $1.95

If I enter the following commission levels:
1 2.11
201 1.95

Will it do what I want it to do?

This commission level is per trade as apposed to a monthly level.

Thanks. Btw, my DOM doesn't display my commission-adjusted PnL in my live account. Though it does in my Sim101 account. Why is that?

edit: And is there a way to edit the commission on past trades?

Some brokers include commissions in PnL values sent, and some do not. Please see the following link which details this:

Unfortunately there is no way to edit executions, but you could remove and add if needed. This is managed through the Right Click screen on the executions tab.
