Visual Studio Unexpectedly Exited

I'm trying to attach Visual Studio to debug code in NT 7 but am getting an error.

"The debugger's worker process (msvsmon.exe) unexpectedly exited. Debugging will be aborted."

Any ideas what this means?

I had to remove a third party vendors assembly because I was getting a failed load message in the trace file. The third party vendor said that is caused by his code because it is compiled in no debug mode.
